ACUVUE® Brand Rebates*Whether you're a new or existing ACUVUE® lens wearer, you can save with our latest rebate offers. Have you recently had an eye exam? Have you purchased ACUVUE® Brand Contact Lenses between January 1, 2010 and June 30, 2010? If the answers are yes, start an ACUVUE® rebate. Rebate is in the form of a Visa® prepaid card. A: You can now check the status of your rebate by logging in to the ACUVUE® Rebate Status page. If you provide your email address on the rebate form, we’ll let you know your submission has been received. You can expect your rebate check in about four to six weeks after you receive your email confirmation. If you have questions about your rebate, contact the ACUVUE® Brand Rebate Center at 1-888-565-8474. Q: Do I need to send in my original receipts with the rebate certificate? A: No. You can submit a photocopy of your ACUVUE® purchase receipt and eye exam receipt. Q: Can I send in purchase receipts from multiple vendors? A: Yes, as long as you purchase the required quantity of ACUVUE® contact lens boxes specified on the rebate form within 90 days after your eye exam. Q: Can I send in more than one rebate for the same purchase? A: No. The rebate is limited to one rebate per customer, per offer, per ACUVUE® Brand purchase, per yearly eye exam. Q: Are there any four box rebates currently available? A: Yes. When you have an eye exam and purchase at least four boxes of ACUVUE® OASYS™ for ASTIGMATISM you will be eligible to receive a $20 rebate. |
